It would appear that Castiel is free of the Leviathans and so I would expect him to come back even though it seemed he is dead. What condition will he be in is the question but for now there are more pressing matters. The Leviathans are individually taking over various people around town and they are very hungry. For now they are working from a hospital and trying to set up their own buffet of humans.
Meanwhile Sam is losing grasp on reality, thinking he may be still in Lucifer's cage and everything that is going on is all illusions from Lucifer himself.
This episode continues directly after the previous and leads right into next weeks. While not as interesting as last week, we are starting to see the season's format. In a way it seems that we will see Sam and Dean battle the Leviathans in a very similar vein when they battled demons. Innocent people being possessed and how to kill them. I can't imagine this can be sustained for the rest of the season but may be a good change from all the angel/heaven storyline for a while.
It was good to see Sheriff Jody Mills from the 5th season's "Dead Men Don't Wear Plaid" which makes sense since this does take place in Bobby's town. Speaking of which, it will be sad not to see Bobby's house again. This place acted as the unofficial headquarters for the Winchesters for several seasons. I don't believe for a second that Bobby was in the house of course and so perhaps he and Castiel will save Sam and Dean from the dangers at the hospital next week.
